A Community Health Assessment (CHA) is a comprehensive, community-driven process used to understand local health needs, identify priorities, and guide action. Granville Vance Public Health, in partnership with organizations and residents across both counties, conducts a CHA every three years so our work reflects the most pressing needs of the community. In the years between assessments, progress is reported annually in State of the County Health (SOTCH) reports.
The 2025 CHA was conducted through a collaborative process led by GVPH with support from community partners and stakeholders. Over several months, this work included community input through surveys, focus groups, and interviews; engagement with local leaders and organizations across multiple sectors; and analysis of over 100 health indicators using state and national data sources — so both data and lived experiences shape a complete picture of community health.
Through this process, two key priority areas were identified as the most pressing opportunities to improve health and well-being in our community:
These priorities reflect both the data and the voices of residents, highlighting the need for improved access to care, prevention strategies, and stronger community support systems.

Findings from the CHA directly inform the Community Health Improvement Plan (CHIP) — a three-year roadmap that outlines specific goals, strategies, and actions to address priority health needs. The current 2026–2028 CHIP focuses on:
Together, the CHA and CHIP serve as the foundation for public health planning and programming, community partnerships and collaboration, grant funding and resource alignment, and long-term improvements in health outcomes. Most importantly, they ensure the health priorities of Granville and Vance counties are driven by the community, for the community. Community input doesn’t stop with the assessment — GVPH will host a Community Health Assessment Symposium in summer 2026 to share key findings, engage residents and partners in shaping CHIP implementation, and strengthen collaboration across the region.
