WHAT IS THE COMMUNITY HEALTH ASSESSMENT?

The Community Health Assessment (CHA) is conducted every three to four years to evaluate the health needs of our community. This process helps to identify key issues, improve services, and ensure resources are directed where they are needed most.

Yes. All responses to the Community Health Opinion Survey are completely anonymous. We do not collect your name, address, or any identifying information

The information you share helps public health officials, healthcare providers, and community organizations better understand what’s working and what needs improvement. Your feedback guides decisions about programs, services, and funding priorities in Granville and Vance counties.

Anyone who lives, works, worships, or attends school in Granville or Vance County is encouraged to take the survey. We want to hear from people of all backgrounds, ages, and neighborhoods—your voice matters.

We’ll analyze the data after the survey and listening sessions are complete. A full Community Health Assessment (CHA) report will be released publicly in early 2026. Key findings will be shared on our website and social media, and used to create a Community Health Improvement Plan (CHIP).
